Wirral End of Life Charter

The new Wirral End of Life Care Charter, setting out twelve expectations that Wirral people may expect to receive when they need End of Life care was officially launched to health and social care providers on Wednesday 20th May 2015.

St George’s Medical Centre is proud to be one of the many organisations that have signed the new Wirral End of Life Care Charter. The charter and a copy of the practice’s pledge can be found below.

Wirral EoLC Charter Information Pack

The charter seeks to show that help is available for people with life-limiting conditions to have the best quality of life possible, for as long as possible, and that their wishes lie at the heart of their end of life care.

The creation of the Charter is a milestone in the progress of quality end of life care on Wirral. It is supported by Wirral End of Life and Palliative Care Partnership Group, and Cheshire and Merseyside Palliative and End of Life Care Clinical Network Steering Group.
