Your Contact Details

It is extremely important that we have your correct contact information.

If you have changed your address, telephone number or email please let us know either by:

• The best way to update your contact details is via our online system.
• If you are not registered please ask one of our Reception staff for assistance
• If you are registered with Patient Access, use the ‘Update Your Details’ tile on the right hand side
• Alternatively you can write to us at:

When sending any correspondence, please include your old information along with your new details (including your name and date of birth). If the changes also apply to other members of your family/household please mention this and include their details as well.

Current patients who move to an address outside of our practice area are eligible to apply for Out Of Area registration. For more information, please navigate to our Practice Area Page under the ‘Patient Info’ tab above or speak to one of our Receptionists on 0151 630 2080.

Please note that if you are changing your name we need to have proof of name change such as Marriage Certificate or Deed Poll.
